Pre Appointment Self Assessment

To make your appointment a more effective use of time, there are a few things I would like you to send in ahead of time.

I am asking for some background information, or as I prefer to call it, History
There is a form below that you can fill in for that, it will automatically land in my inbox.

What is less automated, unfortunately, are the videos and Pictures I will ask for.

Detailed below are the videos and pictures I would like you to send, you may upload them to youtube set to private link which you them email me, or if you have a file sharing service such as Google Drive, Drop box or similar, upload them there and email the link.

I will leave the precise details of how to send up to you, but if you get really stuck, do drop me a line and I'll try help you.
